Foreigner Physical Examination Form
The Foreigner Physical Examination Form must be completed by a licensed physician at an authorized hospital. The form must be fully filled in and include the attending doctor’s signature, the official hospital stamp, and a passport-sized photo of the applicant stamped by the hospital. Incomplete forms, or forms missing any of these elements, will be considered invalid. Please note that the medical examination results are valid for 6 months only, so applicants should ensure the examination is conducted within the required time frame.
Study Plan Proposal
A recent bank statement demonstrating sufficient financial capacity to support tuition fees and living expenses during the study period. The minimum required amount is USD 2,500 or equivalent. The statement must clearly show the account holder’s name and balance.
Certificates of academic awards, scholarships, competitions, or other officially recognized achievements obtained by the applicant. These documents help demonstrate academic excellence and extracurricular involvement.

Passport (Valid for at Least 12 Months)
A clear copy of a valid passport is required. The passport must remain valid for at least 12 months from the date of application to ensure eligibility for enrollment, visa issuance, and residence permit procedures.
Application Form

Non-Criminal Record Certificate
An official Non-Criminal Record Certificate issued by the local police authority, confirming that the applicant has no criminal history. This document is mandatory for admission and residence procedures.

Recommendation Letters
Two letters of recommendation written by full professors or associate professors in the relevant academic field. Each letter must include the professor’s signature, contact phone number, and email address.
